Our incident report forms are extremely versatile. They can be used by teachers in class to report a series of inappropriate behaviour, by lunchtime supervisors and staff managing behaviour around the site and by anyone who needs to report a serious incident.

The incident report forms can be used as a stand-alone, or can be attached to your existing pro-formas. They allow colleagues not only to record the inappropriate behaviour, but also the behaviour management strategies that they used to manage the incident.
